A garage door rides on rollers that run inside a metal track on each side. When something throws that system off, the door jumps the rail. These are the usual reasons.
The most common cause by far. Backing into a half-open door, even lightly, knocks it off the rails and can bend both the panels and the track.
Rollers crack, flatten, or seize up with age. A roller that pops out of the rail lets that corner of the door slip off the track.
A dented rail, or a track that has worked loose from the wall, gives the rollers nowhere to run, so the door binds and jumps out.

Dirt, a small stone, or a hardened lump of old grease in the track can push a roller off the rail as the door travels.

How to prevent it
Most off-track doors can be prevented with a little regular care. The door runs on rollers, hinges, cables, and springs that all wear over time, and a few simple habits keep them running true.
Keep the tracks clear of dirt and debris, and never run the door when something is blocking it. Once a year, have the rollers, hinges, springs, and cables checked and lubricated and the hardware tightened, so a worn part gets caught before it drops the door. And give the door room: do not pull out until it is fully open, since clipping a moving door is one of the fastest ways to knock it off track. A quick annual tune-up costs far less than an emergency garage door off track repair.
